As far as I know this design first appeared as 'Boot Bill Fold' on pages 280/1 of issue 363 of The New Phoenix magic magazine, which was published in 1961.

The article states:

A variant of the design appears as 'Santa's Boot' in 'The Folding Money Book' by Adolfo Cerceda, which was published by The Ireland Magic Co in Chicago in 1963.
