From the context in which this design appears it is most probably made from an oblong which is first pleated lengthwise and then folded at right angles to produce the head.

As far as I know this design first appears in part two of 'The Kindergarten Guide' by Maria Kraus Boelte and John Kraus, which was probably first published by E. Steiger and Company in New York in 1882.

A design called 'The Hatchet', which, from its context is probably the Hammer, is mentioned but not illustrated in 'Practical Kindergarten Lessons for English Infant Schools' by Mrs E. Mortimer was published by Joseph Hughes and Company in London in 1891

As 'A Hammer' in 'Course of Paperfolding' by Eleonore Heerwart, which was published by Charles and Dible in London and Glasgow in 1895.
